Parent/Guardian of Student athletes should complete and sign the PIAA Paperwork (sections 1-5) & the UPMC Consent Form
Student Athletes should bring completed forms along with them the morning of the physical.

Tuesday, June 2, 2026 for Current Grades 9-11*
Wednesday, June 3, 2026 for Current Grades 7-8*
All HS PIAA physicals begin at 7:30 a.m. sharp !
Fall Sports Physicals for Athletes in Current Grade 6 will be held at the Elementary School Nurse’s Office on:
Monday June 1, 2026 for Current Grade 6*
6th grade PIAA physicals begin at 9:00 a.m
